You have been referred for specialized care to an Oral & Maxilofacial Surgeon. To ensure your visit is efficient and productive, we request that you please:

Bring copies of your Dental, Medical, and identification cards.
Bring a list of all prescription and over-the-counter medications you are taking.
As we will be taking radiographs, please be prepared to remove all head and neck jewelry.
